Intake forms, insurance and payment setup, and consent signage for care
Initial Consultation with Dr. D’Amico including your medical history, health goals, and baseline labs/vitals

NEXT 30 DAYS
Overall health check-in around 5-6 weeks into your care plan
Follow up on tests, vitals, etc. along with an adjusted care plan based off of your needs
Sharing of educational materials and support as needed
Sharing questions, feedback, and requests to better tailor your care to you
Personalized messages from your nurses, health coach, and/or physician
Retention & Loyalty
FINAL 30 DAYS
Overall health check-in & progress evaluation around 9-12 weeks into your care plan
